Athletes and parents both agree to the same standards before the first practice. It takes a minute, and it's the part that makes everything else on the deck possible.
Read first, then sign. The Code of Conduct is short and specific — it describes what we'll actually hold your athlete to. Review the policies before completing this form.

By submitting this form, athletes and parents or guardians agree to uphold Meridian's Code of Conduct, behavioral standards, and safety policies, along with USA Water Polo's Rules Governing Conduct and Safety Policies. Failure to meet these standards may result in disciplinary action, including removal from the club.
Both an athlete and a parent or guardian need to acknowledge. Each athlete in a family is submitted separately, so if you have two daughters in the program, complete the form twice.
